Hello, this is Dr. SangYoul Yun, board-certified dermatologist. As we age, the face loses volume, the cheekbones become more prominent, and the face can look longer. In particular, some people lose volume in the lateral cheek so that the cheekbones and jaw protrude while the side of the cheek hollows — a "peanut-shaped" face.

Who needs lateral cheek filler?

In Western faces, hollow lateral cheeks and prominent bone can be a mark of beauty. For East Asian faces, however, a long-looking face and hollow lateral cheeks can make the impression look harsh or older — which many people would rather avoid. In these cases, filler can restore volume to the lateral cheek and make the overall impression softer and more natural.

In the lateral cheek, the deep fat sinks to create a deep groove, and the subcutaneous fat above it gradually descends as well. During lateral cheek filler, the injection passes the masseteric cutaneous ligament — a firm ligament — so when a cannula is used you may feel slight pain or a "pop" at that point.

Choosing the filler

A filler that is too firm tends to clump and form lumps and is harder to use to correct a broad area, so a filler of moderate elasticity is used.

Dosage

Correction varies with severity, but roughly 1–2 cc per side is used. Overfilling can cause the filler to protrude toward the mucosa, so care is needed.

Course

When both lateral cheeks are hollow, the cheekbones and jaw look more prominent and facial asymmetry looks more pronounced. Filling the hollow areas softens the line, giving a younger, gentler impression.

Alternatives to filler

Filler isn't the only way to fill the cheek. Collagen-stimulating treatments such as Sculptra, Radiesse, and Juvelook Volume can also restore facial volume. That said, hyaluronic acid (HA) filler has the advantage that the injector can shape the desired form in real time while watching the patient's condition.
