Ultherapy: 3 Things a Dermatologist Wants You to Know First

A board-certified Seoul dermatologist shares three things worth knowing before Ultherapy: how it differs from other ultrasound lifting, how tips and shot count are chosen, and side effects.

As we age, the jowls settle and the nasolabial folds deepen, and many patients start to feel that only surgery could fix it. But there is a device that can sharpen the contour without surgery. Here are three things worth knowing before Ultherapy — the practical points I go over with patients in clinic.

What Ultherapy is

Ultherapy is an FDA-approved non-invasive procedure that lifts the neck, jaw, and brow. It uses ultrasound to create thermal coagulation points deep in the skin, which regenerates collagen. Among ultrasound lifting devices, it is the one with the strongest and most durable effect. It has been on the market for more than 15 years and has been continuously refined over that time.

Tip 1 — understand what actually makes Ultherapy different

When patients ask why Ultherapy is so widely chosen over similar ultrasound devices, it comes down to four things:

  • It maintains 60–70°C. At this temperature the collagen chains unwind and then re-form, and that remodeling is what produces the lift.
  • Accuracy — energy is delivered precisely.
  • Layered treatment by depth — from the fascia covering the muscle down through the dermis, the whole thickness can be treated.
  • Real-time ultrasound visualization — the operator can see the treatment depth as they work.

Because energy is delivered more accurately and uniformly, the same number of shots can produce a better result — which is what makes the effect stronger and longer-lasting.

Tip 2 — tips and shot count are individualized, not "maximum everything"

Ultherapy typically uses two tips: 4.5 mm and 3.0 mm. The 4.5 mm tip targets the fascia to create the lift, while the 3.0 mm tip treats the septa of the subcutaneous fat. Which tips are used, and in what ratio, changes with the treatment area and how much subcutaneous fat is present — which is exactly why it matters to be treated by an experienced physician.

On shot count: for Koreans, roughly 600 shots for the full face is a common recommendation. But 300–400 shots can also give a sufficient result, so I do not insist on 600 for everyone. Treating from deep to shallow can also bring some improvement in pores and skin texture alongside the lift.

Tip 3 — know the temporary side effects

After Ultherapy, a temporary linear red mark can appear where energy was delivered more superficially; this usually resolves within about 24 hours. Treating around the mouth carries a risk of touching a nerve, which can cause temporary numbness, so that area calls for care. Choosing a genuine Ultherapy device and an experienced dermatologist is the best way to keep both the result and the safety margin where they should be.
