Pain-Free Ultherapy & Thermage with Nitrous Oxide

The comfort question patients always ask

"Does Ultherapy hurt?" and "Is Thermage painful?" are among the most common questions international patients ask before booking. Both procedures use energy-based technology — microfocused ultrasound for Ultherapy, monopolar radiofrequency for Thermage — that can cause meaningful discomfort during treatment. This is normal and expected — but it shouldn't be a reason to avoid effective lifting procedures.

Until recently, the two options were to tolerate the discomfort or receive IV sedation ("sleep anesthesia"). IV sedation carries its own downsides: a needle, recovery grogginess, fasting requirements, inability to drive, and documented risks that make it overkill for a non-surgical procedure. A third option — medical-grade nitrous oxide — has emerged as a safer, more practical middle ground.

To provide safer and more comfortable treatments, we introduced the Matrx MDM nitrous oxide / oxygen sedation system manufactured by Porter Instrument. Nitrous oxide (N₂O) has been used safely in dentistry and medicine for over 150 years, and clinical research increasingly supports its role in cosmetic dermatology.

A 2018 systematic review published in Dermatologic Surgery reviewed studies on nitrous oxide use across dermatologic procedures and reported promising results for procedural pain management, finding that current literature provides some evidence that N₂O is effective at providing analgesia for many dermatologic procedures; the authors explicitly noted the need for additional randomized controlled trials before broader clinical recommendations. A 2021 study on a novel N₂O delivery system for cosmetic procedures confirmed that patients reported significantly lower pain scores during energy-based treatments when nitrous oxide was used, with no serious adverse events.

Even for the discomfort of local anesthetic injections themselves, a 2019 clinical trial found that nitrous oxide pre-treatment significantly reduced pain from lidocaine injection — meaning patients are more comfortable from the very first step of the procedure.

How it compares to IV sedation

IV sedation (propofol or similar) is standard for major surgery but is an unusually heavy intervention for a non-surgical cosmetic procedure. The Dermatologic Surgery systematic review described N₂O as a practical adjunct for dermatologic procedures that avoids the recovery time, monitoring requirements, and drug-metabolism considerations of IV sedation, while also calling for more randomized trials to refine clinical guidance.

Who benefits most from nitrous oxide?

Based on the clinical evidence, the MATRx system is especially helpful for patients who:

  • Feel uneasy about IV sedation but still want effective pain management
  • Want to stay conscious and communicate with the doctor during the procedure
  • Have a lower pain threshold but don't want to skip effective treatments
  • Are visiting from overseas and need to minimize total recovery time
  • Have concerns about sedation-related drug interactions or medical history

What to expect during your visit

When you arrive for your Ultherapy or Thermage FLX session, the dermatologist will discuss whether nitrous oxide is appropriate for your case (it's contraindicated for certain conditions like severe COPD, first-trimester pregnancy, and recent ear surgery). If it's appropriate, a small nasal mask is placed over your nose, and you'll begin to feel relaxed within a few minutes. The procedure is performed as normal. Once the mask is removed, the effects wear off within 5 minutes because nitrous oxide is eliminated through exhalation rather than metabolized by the liver. You can walk out of the clinic on your own and resume your day.
